The Wellness Plan

You finished therapy. You felt good. The exercises went from every day, to when you remembered, to never. And right about the time you had forgotten you ever had a problem, the problem came back. That is not a character flaw and it is not laziness. It is what happens when a program ends and nothing takes its place.

01After Therapy

Feeling better and staying better are not the same project.

Formal physical therapy ends when you reach your goals, and that is the right place to end it. What it cannot do is hold the gains for you. Strength, mobility and control are kept by continuing to load them, and a program written for the version of you that was still in pain stops being the right program the moment you are not. The wellness plan is the part after the part. Ongoing progression, someone qualified watching how you move, and a standing reason to keep showing up.

The gap that undoes good rehab

Discharge day is the most dangerous day in rehab. You feel fine, so the home program quietly gets demoted, and nothing bad happens for a while, which is exactly the trap. The tissue that got you here is still the tissue that got you here. Recurring pain is usually not a new injury. It is the old one, unloaded and unattended.

Progression, not repetition

Doing the same discharge exercises forever stops working, because your body adapted to them and then stopped changing. On a wellness plan the work moves with you. Load goes up, positions get harder, and the movements start to look more like the things you actually want to do: the garden, the ladder, the grandkids, the golf swing.

Measured, not assumed

You get reassessed periodically rather than being left to guess. Same tests, same measurements, compared against where you were. If something has slipped we catch it while it is still small and adjust. If everything is holding, you get told that, which is worth hearing. It is the same thinking behind an annual physical therapy exam, applied continuously.

Accountability, said out loud

Most people do not need more information about exercise. They need a time on a calendar and a person who notices when they do not turn up. That is not a weakness, it is how nearly everyone works. Having somewhere to be, and someone who knows your history and will correct your form, is the difference between a plan you have and a plan you do.
