Patient Paperwork
There is nothing to download here. We do not use printed PDF packets. Instead we use short pre-exam forms you fill in online, one for each body area, so your therapist has read your story before you sit down. Filling one in is optional. If you would rather answer the questions by phone with our front desk, or simply do it when you arrive, that is completely fine and it will not slow your visit down.
Nothing to print. Nothing to download.
The forms below open in your browser and are answered on screen. Pick the one that matches what is bothering you, work through the questions at your own pace, and your therapist reads it before you walk in, which means your first appointment starts with a hands-on evaluation rather than a clipboard. Skip it if you would rather just talk it through in person. Nobody is turned away for arriving with nothing.
New Patient Pre-Exam
Start here if we have not seen you before. Your contact details, what is bothering you, how long it has been going on, and what you want to get back to doing.
Open Form →Back Pain Pre-Exam
For low back pain, sciatica, and anything that travels down the leg. Where it hurts, what makes it worse, and what you have already tried without much luck.
Open Form →Neck Pre-Exam
For neck pain and stiffness, headaches that start at the base of the skull, and pain or tingling running into the shoulder, arm or hand.
Open Form →Balance & Falls Pre-Exam
For dizziness, unsteady footing, or a fall you would rather not repeat. It asks about your history of falls and what tends to set the wobble off.
Open Form →Bring your prescription or referral if a doctor gave you one, a photo ID or driver's licence, a list of your medications with the dosage and how often you take them, your personal calendar so we can book the rest of your visits before you leave, your insurance cards or your Worker's Compensation or auto claim information, and copies of any X-ray, MRI or CT reports you have. Missing something? Come anyway and we will work around it. Kansas direct access means most people start without a physician referral, and where a plan does require one we check that for you before your first visit. There are more pre-exam forms, sorted by body area and by clinic, on our patient forms page, and the plans we accept and file are listed on our insurance page.
