Privacy Policy
Effective Date: August 13, 2026
Abilene Physical Therapy and Sports Rehab ("we," "us," or "our") operates two physical therapy clinics in Kansas and the website at abileneptsportsrehab.com. This policy explains what information this website collects, why we collect it, who else can see it, and the choices you have. It covers the website. It does not replace the separate Notice of Privacy Practices that governs your medical record once you become a patient, which is described further down this page.
Information We Collect Through This Site
Most of this website can be read without giving us anything at all. You do not need an account, and we do not ask you to register.
When you submit the appointment request form, we collect what you type into it: your name, your phone number, your email address, and whatever you choose to tell us about your pain, your injury or your question. That last field is free text, so please share only what you are comfortable sharing at this stage. A short description is genuinely enough. You do not need to write out your medical history to get a call back, and we would rather you did not send diagnoses, test results or images through a web form.
We also receive the ordinary technical information that any web server receives, such as your IP address, your browser and device type, the pages you looked at, and the site or search that sent you here. This is used to keep the site working and to understand which pages people actually find useful.
Text Messages and SMS Consent
The consent checkbox on the appointment form is separate from the rest of the form and it is optional. Ticking it means you agree to receive text messages from Abilene Physical Therapy and Sports Rehab about your appointment. Message and data rates may apply. Message frequency varies.
Reply STOP to any message to opt out, and we will stop texting you. Reply HELP for assistance, or contact the clinic directly using the details at the bottom of this page. Opting out of text messages does not affect your care, your ability to book, or anything else. We will simply call or email you instead. We do not sell or share your phone number with third parties or affiliates for their own marketing purposes, and mobile opt-in consent is never shared with anyone for that purpose.
How We Use Your Information
We use what you send us to call you back, to schedule and confirm your appointment, to verify your insurance benefits before your first visit, to answer the question you asked, and to follow up about your care. We use the technical information to maintain, secure and improve the site. We do not sell your personal information, and we do not rent or trade it.
How We Share Your Information
We share your information only where it is necessary: with the service providers listed below who help us run this website and receive form submissions, with your insurer or a claims administrator where that is required to bill for your care, and where the law requires it or permits it, for example in response to a valid legal process. Service providers receive only what they need to do their job and are not permitted to use your information for their own purposes.
Health Information and HIPAA
Once you become a patient, your medical record is protected health information and is handled under the Health Insurance Portability and Accountability Act. How that record is used, disclosed and protected is set out in our Notice of Privacy Practices, which is a separate document you can request at either clinic or by calling us. That notice, not this web policy, governs your clinical record.
Information you send through this website before you become a patient is treated with care, but a public web form is not a secure medical channel. Please do not use it to send sensitive health details. If you need to discuss something sensitive, call us and we will take it from there.
Third-Party Services on This Site
This site is hosted by Netlify, which also receives and stores submissions from our appointment form before they reach our front desk. Location pages embed Google Maps so you can get directions, and Google can receive information about that embed when the map loads. Our patient reviews are displayed through an Elfsight widget that pulls our public Google reviews. Typefaces are served by Google Fonts. Each of these providers operates under its own privacy policy, and loading a page that uses one of them may involve a request to that provider.
Cookies
We use a small number of cookies and similar technologies to keep the site functioning, remember basic preferences, and understand traffic in aggregate. Third-party components such as the map embeds and the reviews widget may set their own cookies. You can block or delete cookies in your browser settings. Parts of the site, particularly embedded maps and reviews, may not display properly if you do.
Data Security and Retention
We take reasonable administrative and technical measures to protect the information we hold, and we limit who inside the clinic can see it. No website or transmission over the internet can be made completely secure, and we will not claim otherwise. We keep website enquiries only as long as we reasonably need them to respond to you and to meet our recordkeeping obligations, and clinical records are retained for the period required by Kansas law and by our professional obligations.
Your Rights and Choices
You may ask us what information we hold about you, ask us to correct it, or ask us to delete it, subject to the records we are legally required to keep. You may opt out of text messages by replying STOP, and out of email at any time by telling us. You may decline to fill in any field on any form on this site. None of these choices will affect the care you receive from us.
This site is intended for adults. We do not knowingly collect information online from children. Where a patient is a minor, a parent or guardian handles the paperwork and communication with us directly.
Changes to This Policy
If we change how we handle information, we will update this page and revise the effective date shown at the top. Continued use of the site after a change means you accept the updated policy.
