New Patient Pre-Exam Form in Abilene
First appointments go one of two ways. Either you spend the front half of it reconstructing your own history from memory, or you walk in with it already sorted. This page is how you get the second version. Everything your therapist will ask at our Abilene clinic, listed in advance.
What your new patient pre-exam covers.
An evaluation is mostly detective work. Your therapist tests, measures and watches you move, but the fastest route to the right answer is usually something you say in the first few minutes. That is what this covers.
Read it before you come in and answer whatever you can. There is nothing to submit on this page, nothing to print, and no wrong answers. If a question does not apply to you, skip it. When you are ready for a time, use the booking form further down the page.
Why you are coming in
The reason for the visit in your own words, not medical ones. Where it is, when it started, and whether there was a moment you can point to or whether it simply arrived. Then whether it has been getting better, worse, or holding steady since, and how it has changed since that first week.
What we treat →What makes it better, what makes it worse
Sitting, standing, walking, lifting, sleeping, the first hour of the day, the last hour of a shift. Heat, ice, movement, rest. Tell us what reliably winds it up and what reliably calms it down, and name the things you have quietly stopped doing because of it.
Request an appointment →Your medical picture
Treatment you have already had for this, including imaging, injections and any surgery, plus what changed after each. Then the wider picture: your current medications with dosage and frequency, past surgeries, and other conditions such as diabetes, heart or blood pressure issues, osteoporosis or pregnancy.
Insurance we accept →What you want back
This is the one people undersell. Not “less pain”, but the actual thing: the stairs at your daughter’s house, a full shift without the brace, lifting a grandchild, sleeping through the night, being on the mower again. Name it plainly, because it becomes the goal we measure everything else against.
Call (785) 263-3646 →What to bring to your first visit
- Your prescription, if a physician has already written you one
- Your driver’s licence or another photo ID
- Your insurance cards, or your Worker’s Comp or auto claim information
- A list of your medications with the dosage and how often you take them
- Copies of any X-ray, MRI or CT reports connected to this problem
- Your personal calendar, so we can schedule the rest of your visits before you leave
Wear something you can move in. Kansas direct access means most patients can start physical therapy without a physician referral, so you can call us and be evaluated. Some insurance plans do still require one for coverage, so we verify your specific plan before your first visit and tell you exactly where you stand, rather than letting you find out afterwards.
