Annual PT Exam Form

The exam works better when you have already thought about the answers. Nothing on this page is a test and nothing here is required. It is simply what your therapist is going to ask, written down so you can think it over at the kitchen table instead of in a waiting room.

01Before Your Annual PT Exam

What we will ask you.

None of this is complicated. It is the information that turns a movement screen into a screen about your life rather than a generic checklist.

  • What you are doing now: work, sport, training, walking, lifting grandchildren, all of it
  • Anything nagging: the shoulder that aches on the last set, the hip that talks to you on the stairs
  • Past injuries and surgeries, including the ones you believe healed fine
  • Any falls or near falls in the past year, including the ones you did not mention to anyone
  • Your current medications, with dosage and frequency
  • What you want to still be doing 5 years from now, said out loud and specifically

What to bring.

  • Your prescription, if a physician gave you one
  • A photo ID or driver’s licence
  • A list of your current medications, with dosage and frequency
  • Your personal calendar, so any follow up can be booked before you leave
  • Your insurance cards, or your Worker’s Compensation or auto claim information
  • Copies of any X-ray, MRI or CT reports

If writing things out is not your thing, do not. You can answer every one of these by phone before your visit, or out loud with your therapist once you arrive. Nobody is going to hand you a clipboard and walk away. If you want the fuller picture of what the exam covers first, read the Annual PT Exam page.
