Lower Extremity Pre-Exam Form in Chapman
You have probably been favouring that leg long enough that you have stopped noticing you do it. Here is what your therapist will ask about your hip, knee, ankle or foot at our Chapman clinic, written down in advance so the first visit can be spent examining rather than catching up.
What your lower extremity pre-exam covers.
The leg works as one chain. A knee that aches is often a hip that quit pulling its weight, and a foot that will not settle is often an ankle that never properly healed years ago. So this pre-exam asks about the whole limb, not only the spot you would point to.
Have a read before you come in. Answer what you can and leave the rest for your therapist to fill in. Nothing is submitted on this page. When you want an appointment, use the booking form further down.
Which joint, and how it started
Hip, knee, ankle, foot, or several at once, and which side. Then how it began: one clear moment, a twist, a landing, a curb that was lower than you expected? Or no event at all, just weeks of it getting louder? Those two stories point us in different directions.
Hip & knee pain →Swelling, giving way and locking
Does it swell, and does that swelling arrive within the hour or by the next morning? Has the knee ever buckled with no warning? Does anything catch, lock, or refuse to straighten out? Clicking by itself is usually just noise. Clicking that hurts, or a joint that jams, we want to hear about.
Foot & ankle pain →Stairs, mornings and distance
Stairs on the way up, on the way down, or both. The first step out of bed before the foot has warmed up. Standing after a long sit. How far you get on a walk before it starts complaining, and whether kneeling, squatting or climbing out of the truck has become a two stage job.
Request an appointment →Footwear, past surgery and the goal
What is on your feet all day, boots, trainers, work shoes, and whether the tread wears down on one edge. Any braces or orthotics. Every previous injury or operation on that limb, no matter how far back. And the part that matters most: the specific thing you want to be doing again.
Call (785) 922-4016 →What to bring to your first visit
- Your prescription, if a physician has already written you one
- Your driver’s licence or another photo ID
- Your insurance cards, or your Worker’s Comp or auto claim information
- A list of your medications with the dosage and how often you take them
- Copies of any X-ray, MRI or CT reports for that hip, knee, ankle or foot
- Your personal calendar, so we can schedule the rest of your visits before you leave
Wear or bring shorts, and bring the shoes you live in. Kansas direct access means most patients can start physical therapy without a physician referral. Some insurance plans still require one for coverage, so we verify your specific plan before your first visit and tell you exactly where you stand.
